What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of substance use by embedding biomarkers within the strands as they grow. When collected near the scalp, hair can offer a detection window of up to around 3 months for alcohol and drugs. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment consisting of roughly 200 hair strands (similar in diameter to a #2 pencil) located closest to the scalp will yield 100mg, which is the optimal sample size for screening and verification. For EtG, additional tests, or panels exceeding 10, it is advisable to collect 150mg. It is recommended to use a jeweler’s scale for measuring the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. When discussing head hair, it refers exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first processed into the lab’s system. This step involves checking that the sample is properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening involves a rapid preliminary test for substances of abuse. Although Screening cost-effectively excludes drug use across most samples, a positive result must be confirmed to stand in legal proceedings. Any samples with a presumptively positive result in Screening must undergo further verification.

If a sample tests presumptively positive at the Screening phase, additional hair is taken from the initial sample for Extraction preparation. At this point, substances are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), highlighting the complexity of conducting hair drug screening.

Confirmation of any positive result from Screening is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are presumptive positive are washed before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, is reviewed following both the CAP (College of American Pathologists) Hair designation standards and accreditation to ISO / IEC 17025 standards.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Remarkable advancements in sample preparation, detection, and identification of drug compounds have been presented. The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
