When gathered near the scalp, hair can offer a detection period of up to approximately three months for various substances. Hair is straightforward to gather, relatively hard to alter, and easy to transport.

An ideal sample size for diagnostic and confirmatory purposes is a 1.5-inch segment of roughly 200 hair strands (about the diameter of a #2 pencil) closest to the scalp, which amounts to 100mg of hair. For EtG, supplemental tests, and/or panels above 10, a quantity of 150mg is advised. It's beneficial to measure the specimen on a jeweler’s scale. If scalp hair is not accessible, a comparable volume of body hair can be procured. The term head hair signifies scalp hair exclusively. Body hair pertains to all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory process of obtaining a drug test outcome consists of four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial induction of a specimen into the lab's records. This involves ensuring the sample's proper sealing and delivery, assigning a random LAN (Laboratory Accessioning Number), and completing any ancillary data entry absent in an electronic chain of custody framework.

Screening performs an initial rapid assessment for substance misuse. Although Screening is a budget-friendly method for dismissing substance usage in most samples, a positive outcome here requires further confirmation for legal acceptability. Any samples showing presumptive positives in Screening necessitate follow-up confirmation.

Should a sample come back as presumptive positive during Screening, additional hair is then extracted from the original sample for Extraction. During this phase, substances are extracted from the hair at a notably lower concentration compared to alternative methods (e.g., urine or oral fluid), making hair drug screening the most rigorous methodology to execute.

Certification of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumed positive results undergo a washing process pre-certification, as required. The complete laboratory workflow from Accessioning to Certification is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
