A sample measuring 1.5 inches of approximately 200 hair strands (roughly akin to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for analysis and verification. For EtG, additional tests, or panels over 10, a 150mg sample is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair isn't an option, a comparable volume of body hair can be used. By head hair, we mean only scalp hair. Body hair encompasses other types of hair (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the first-stage handling of a specimen within a lab's system. This step includes ensuring the sample's seal and shipment integrity, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data not entered through an electronic chain of custody process.

Screening is a preliminary evaluation for substances of abuse. Though Screening provides an economical means to exclude usage of substances in most specimens, any positive results must be confirmed for court acceptability. Samples that test presumptively positive in Screening necessitate further confirmation.

In cases where a sample indicates a presumptively positive result during Screening, additional hair is retrieved from the initial sample set and prepped for Extraction. The process extracts substances from hair at much lower concentrations than other standard methods (e.g., urine or oral fluid), making hair drug screening one of the most intricate methodologies.

GC/MS, GC/MS/MS or LC/MS/MS is employed to confirm any screening results that are positive. All samples that are presumptively positive are cleansed prior to confirmation as needed. The comprehensive lab process, from Accessioning through Confirmation, undergoes evaluation under both the CAP (College of American Pathologists) Hair classification and the certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
