Nevertheless, for drug use in the preceding 5 days, urine tests remain most precise.

What it is, How it works

Hair testing is increasingly acknowledged as an impactful method for detecting drug and alcohol consumption. Hair offers an extended timeline of drug and alcohol use by capturing biomarkers within the strands of the hair as it grows. When obtained near the scalp, hair can provide up to around a 3-month detection period for drugs and alcohol. Collecting hair is straightforward, somewhat resistant to tampering, and simple to transport.

A 1.5-inch sample consisting of about 200 hair strands (approximately the size of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional screenings, and/or tests exceeding 10 panels, a 150mg specimen is advised. Measuring the specimen on a jeweler's scale is recommended. If scalp hair is not accessible, an equivalent quantity of body hair can be used. When referencing head hair, we mean scalp hair alone. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample's initial integration into a laboratory's system. This process includes verifying that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry required that is not supplied by an electronic chain of custody system.

Screening entails an initial rapid analysis for abusive drugs. While Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be substantiated to be legally valid. Samples that are presumptively positive during Screening require a secondary confirmation.

Should a sample yield a presumptive positive result in the Screening step, further hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are extracted from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of any positive screening results is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Note: While commonly called "hair follicle tests", the analysis focuses on the hair strand, not the hair follicle beneath the scalp.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
