Hair collection is straightforward, somewhat hard to tamper with, and easy to transport.

For optimal testing and confirmation, a 1.5-inch section of approximately 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg of hair—the ideal sample. For EtG, additional tests, or panels over 10, 150mg is suggested. Utilizing a jeweler’s scale to weigh the hair is advisable. In cases where scalp hair is not obtainable, a comparable amount of body hair serves as a substitute. When discussing head hair, it specifically pertains to scalp hair, whereas body hair encompasses all other types (such as facial and axillary hair).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a lab’s database. This step ensures that the sample was appropriately sealed and transported, assigns it a unique LAN (Laboratory Accessioning Number), and completes any supplemental data entry absent from an electronic chain of custody system.

Screening performs a swift preliminary check for illicit substances. Although Screening efficiently rules out drug usage in most samples and is economical, a positive screen mandates confirmation for legal admissibility. Any samples deemed preliminarily positive during Screening necessitate further verification.

When a sample turns up presumptively positive in Screening, additional hair is extracted from the primary specimen for Extraction. Here, drugs are derived from hair at lower concentrations compared to other methods (such as urine or saliva), making hair drug screening a more challenging process than other methodologies.

Positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that test presumptive positive are appropriately washed prior to Confirmation as needed. The comprehensive laboratory workflow from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair criteria and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
