Hair is straightforward to collect, moderately tamper-resistant, and convenient to transport.

A sample of 1.5 inches with around 200 hairs (comparable to a #2 pencil) close to the scalp yields 100mg, which is the preferred amount for analysis. For EtG, additional tests, or items over 10 panels, 150mg of the sample is advised. We suggest utilizing a jeweler’s scale to measure. In cases where scalp hair is not available, equivalent quantities of body hair are acceptable. When mentioning head hair, it pertains solely to scalp hair. Body hair includes all other varieties (facial, axillary, etc.).

Process Summary

The laboratory analysis of a drug test encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's entry into the lab system. This comprises confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary manual data entry missing from an electronic chain of custody.

Screening consists of an initial assessment for substance abuse. While Screening is an economical method to eliminate drug usage in most samples, any positive finding must be confirmed for legal admissibility. Samples that are tentatively positive during Screening require further confirmation.

If a sample is tentatively positive during Screening, additional hair is extracted from the initial sample to prepare for Extraction. Here, substances are isolated from the hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair testing is the most challenging technique to execute.

Confirmation of any positive result from Screening occurs through methods like GC/MS, GC/MS/MS, or LC/MS/MS. Every tentative positive sample is cleaned before confirmation, as necessary. The comprehensive process from Accessioning to Confirmation is audited with both CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
