In cases of EtG, add-ons, or tests exceeding 10 panels, 150mg is advised. We propose measuring the specimen with a jeweler's scale. Should scalp hair be unobtainable, a proportional quantity of body hair can be used. "Head hair" specifically refers to scalp hair, while "Body hair" encompasses all other hairs (such as facial or axillary).

Process Overview

The quartet of key steps in lab processing of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering a sample into a lab's system at the outset. This stage encompasses ensuring the sample was securely sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and filling in any absent data not offered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs. While Screening presents an economical method for excluding drug use in most cases, a positive screen necessitates confirmation to be court-admissible. Samples showing potential positives in Screening must undergo secondary confirmation.

For presumptive positives in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. This phase involves pulling drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug testing is among the most challenging methodologies to execute.

Positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples bearing presumptive positives are pre-washed as necessary before confirmation. The full lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
