A sample measuring 1.5 inches from around 200 strands of hair (roughly the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is optimal for screening and confirmation purposes. For EtG, additional tests, or panels exceeding 10, a specimen quantity of 150mg is advisable. We suggest weighing the specimens using a jeweler’s scale. In instances where scalp hair is unavailable, an equivalent volume of body hair can be collected. Reference to head hair exclusively pertains to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

The Accessioning process entails the initial integration of a sample into the laboratory’s system. This process includes confirming the sample was appropriately sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not available via an electronic chain of custody system.

Screening offers a preliminary assessment for substance abuse. Whereas screening offers an economical means to dismiss drug use for the vast majority of samples, a positive result from screening necessitates confirmation to be recognized in a legal setting. Any samples yielding a presumptive positive outcome during Screening require subsequent confirmation.

For samples exhibiting presumptive positive results during the Screening phase, additional hair is extracted for the Extraction phase. During this phase, drugs are withdrawn from hair at significantly lower concentrations than possible through alternative methods (such as urine or oral fluid), establishing hair drug testing as one of the most challenging methodologies to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positive outcomes are cleansed prior to confirmation as warranted. From Accessioning through Confirmation, the entire laboratory workflow adheres to the CAP (College of American Pathologists) Hair standards and accreditation under ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
