For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advisable. Using a jeweler's scale to measure the specimen is suggested. If scalp hair is lacking, a comparable quantity of body hair may be utilized. When discussing head hair, we are specifically referring to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning means the initial incorporation of a sample into a lab's framework. This entails confirming the sample was sealed and dispatched properly, assigning an arbitrary LAN (Laboratory Accessioning Number), and finishing any remaining data input not covered by an electronic chain of custody system.

Screening is a preliminary swift evaluation for drugs of misuse. Although Screening is a budget-friendly method to eliminate drug use in most samples, a positive screen must be verified for court approval. Any samples deemed presumptively positive in Screening mandate a subsequent confirmation.

When a sample is presumptively positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. At this point, drugs are isolated from hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), hence hair drug screening is the most challenging method to execute.

Verification of any positive screen outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as needed. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair category and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
