For EtG, add-ons, or tests exceeding 10 panels, 150mg of hair is advisable. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair isn’t available, a comparable amount of body hair can be used. "Head hair" refers specifically to scalp hair, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample into a lab's system. This entails ensuring that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody system didn’t cover.

Screening conducts an initial rapid check for drugs of abuse. Although Screening is cost-efficient for eliminating drug use in most samples, a positive result requires confirmation to be valid in a legal setting. Any samples indicating a positive result in Screening need secondary confirmation.

If a sample shows a positive indication during Screening, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (like urine or oral fluid), which is why hair drug screening is a more complex methodology.

Confirmation of a positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples presumed positive are washed as necessary. The laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
