What it is, How it works

When taken near the scalp, hair can offer nearly a 3-month detection period for substances including alcohol. Hair is straightforward to collect, relatively hard to tamper with, and convenient to transport.

A 1.5-inch segment of around 200 hair strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, or tests beyond 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not available, a similar quantity of body hair can be used. References to head hair imply scalp hair only. Body hair encompasses all other hair types (such as facial and axillary).

Process Overview

The four key stages in the laboratory processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial intake of a sample into the laboratory’s system. This phase ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry that is not provided by an electronic chain of custody system.

Screening consists of a preliminary check for drugs of abuse. Although Screening is cost-effective for ruling out drug use on most samples, a positive result requires confirmation for court admissibility. Samples that test presumptively positive in Screening require additional confirmation.

If a sample tests presumptively positive during Screening, more hair is drawn from the original specimen and prepared for Extraction. In this phase, drugs are extracted at a much lower concentration from hair compared to other methods (e.g., urine or oral fluid), thus making hair drug screening the most challenging method to execute.

Confirmation for any positive result from screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and accredited under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
