For EtG, additional tests, and panels exceeding 10, a 150mg sample is suggested. Using a jeweler’s scale is recommended for specimen weighing. In cases where scalp hair is unavailable, a similar quantity of body hair is acceptable. Referencing head hair pertains only to scalp hair, while body hair encompasses other types (facial, axillary, etc.).

Process Overview

The drug test result process in a lab encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the lab’s system. This step ensures the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry omitted by an electronic chain of custody system.

Screening serves as a preliminary check for substance misuse. While Screening is an economical method for eliminating drug presence in most samples, any positive result requires confirmation for courtroom validity. Samples that are presumptively positive in Screening necessitate subsequent confirmation.

When a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this stage, drugs are retrieved from hair at much reduced concentrations than through other approaches (e.g., urine or oral fluid), contributing to hair drug testing’s complexity.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are washed prior to confirmation as required. The entire lab workflow from Accessioning through Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation alongside the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
