What it is, How it works

When taken from near the scalp, hair can reflect alcohol and drug usage for up to three months. It is easy to collect, resistant to tampering, and simple to mail.

A sample measuring 1.5 inches and consisting of about 200 hair strands (similar to the width of a #2 pencil) from the scalp yields 100mg, which is the ideal amount for both screening and confirming. For tests like EtG, additional testing, or panels greater than 10, 150mg is preferred. Use a jeweler’s scale for precise measurement. If scalp hair isn't available, collect an equivalent amount of body hair. When discussing head hair, we mean scalp hair only, whereas body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The main stages in the lab processing of drug tests are Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial handling of a sample within a lab’s framework. This includes ensuring the sample's secure seal and shipment, assigning it a random LAN (Laboratory Accessioning Number), and finishing any extra data input absent from an electronic chain of custody system.

Screening is a preliminary examination for substance abuse. Though it’s a cost-efficient means to rule out drugs for most samples, a positive must be confirmed for court validity. Any presumptive positives found in Screening require additional confirmation.

Should a sample test as presumptively positive during Screening, more hair is sourced from the initial sample and readied for Extraction. In Extraction, drugs are isolated from hair at much lower concentrations than other methods (like urine or oral fluid), making hair testing a challenging process.

Confirmatory testing of positives from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation if necessary. The lab procedure from Accessioning to Confirmation undergoes reviews under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
