For EtG, add-ons, and/or tests exceeding 10 panels, gathering 150mg is advised. It's beneficial to weigh the sample using a jeweler’s scale. If head hair is sparse, an equivalent amount of body hair is permissible. "Head hair" specifies scalp hair exclusively, while "body hair" encompasses other hair varieties (facial, axillary, etc.).

Process Overview

The essential procedures in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory's infrastructure. This entails verifying that the sample was sealed and shipped accurately, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient method to exclude drug usage in most samples, a positive result must be confirmed for court admissibility. Samples with a positive indication in Screening require further confirmation.

If a sample is preliminarily positive during the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations relative to other methods (such as urine or oral fluid), highlighting why hair drug screening presents significant challenges.

Confirmation of any positive Screening is carried out utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are cleansed before confirmation as needed. The entirety of the laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair categorization and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
