For EtG tests, additional panels, or tests exceeding 10 panels, a specimen of 150mg is advisable. We suggest measuring the sample's weight on a jeweler’s scale. In cases where scalp hair is unavailable, a comparable volume of body hair may be utilized. When discussing head hair, we're specifically referring to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The core procedures in the lab-based processing of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial procedures for a sample entering a lab's system. This step includes ensuring proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any ancillary data input not covered by an electronic chain of custody system.

Screening involves a preliminary assessment for drug presence. Though Screening is a cost-efficient strategy for excluding drug use in most samples, a positive screening result must be confirmed to hold in a legal setting. Any samples with a presumptive positive in Screening require subsequent confirmation.

Should a sample return a presumptive positive in the Screening phase, additional hair is taken from the initial sample for the Extraction process. At this stage, substances are isolated from hair in much lower concentrations than when using alternative methods (such as urine or oral fluid), underscoring why hair drug testing is a challenging method to execute.

Confirmation of positive Screening outcomes is conducted using techniques like GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo washing before confirmation, as needed. The entire lab procedure from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and follows ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
