For EtG, additional testing, and/or panels exceeding 10, 150mg of the sample is advised. It's recommended to measure the sample on a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair may be gathered. When referencing head hair, it pertains solely to scalp hair. Body hair denotes other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into the laboratory’s system. This involves verifying that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not provided by an electronic chain of custody system.

Screening involves a preliminary swift check for substances of concern. Although Screening is a cost-effective way to eliminate drug use on most samples, a positive screen must be confirmed to be legal in court. Any samples that are flagged in Screening as positive require additional confirmation.

If a sample shows positive indications in the Screening phase, further hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a considerably lower concentration than with other procedures (e.g., urine or oral fluid), which is why hair drug screening is the most complex process to conduct.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
