A 1.5-inch portion consisting of roughly 200 hairs (comparable in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for testing and confirmation. For EtG, supplements, and/or examinations beyond 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be collected. When mentioning head hair, we refer solely to scalp hair. Body hair pertains to all other varieties (facial, axillary, etc.).

Process Overview

The four fundamental stages involved in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a lab’s system. This comprises verifying that the sample was securely sealed and shipped correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry not offered by an electronic chain of custody system.

Screening encompasses an initial rapid assessment for drugs of abuse. While Screening is a cost-effective means to eliminate drug usage in the majority of samples, a positive screen requires confirmation to be court-admissible. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is deemed presumptively positive during the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. At this juncture, drugs are extracted from hair at a substantially lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most intricate methodology to perform.

Confirmation of any positive screening outcomes is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
