What it is, How it works

Hair analysis is now recognized as an effective means for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the fibers of the hair as it grows. When sourced near the scalp, hair offers a detection window of up to roughly three months for alcohol and drugs. Hair collection is straightforward, relatively tamper-proof, and convenient for shipping.

A 1.5-inch sample of approximately 200 hair strands (about the breadth of a #2 pencil) near the scalp delivers 100mg of hair, the optimal quantity for both screening and verification. For EtG, additional tests, and/or assessments above 10 panels, it's advisable to collect 150mg of the hair sample. We suggest using a jeweler’s scale to weigh the hair. If scalp hair is inaccessible, the equivalent amount of body hair can be used. When mentioning head hair, we specify scalp hair only. Body hair comprises all other hair types (facial, axillary, etc.).

Process Overview

The principal steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab’s database. This step involves confirming that the sample was correctly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing further data input beyond what is provided by an electronic chain of custody system.

Screening serves as an initial swift assessment for drugs of abuse. Though Screening offers a cost-efficient way to exclude drug use for most samples, a positive screen needs to be verified for court admissibility. Samples that test presumptively positive during Screening require a secondary confirmation.

Should a sample test presumptively positive in Screening, additional hair is extracted from the original sample and set up for Extraction. During this stage, drugs are extracted at considerably lower concentrations than other methods (e.g., urine or oral fluid), highlighting why hair drug analysis is the most challenging methodology to execute.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is cleansed before confirmation as needed. The full lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and the accreditation for ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
