What it is, How it works

A 1.5-inch segment consisting of roughly 200 strands of hair (comparable to a #2 pencil) closest to the scalp yields 100mg of hair, the optimum amount for evaluation and confirmation. For EtG, supplements, and/or tests exceeding 10 panels, 150mg of the sample is advised. We advocate using a jeweler’s scale for weighing the sample. If scalp hair is not obtainable, a similar amount of body hair may be collected. When we mention head hair, we strictly mean scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory assessment of a drug test outcome follows four significant steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary processing of a sample within a laboratory’s framework. This stage ensures the sample was properly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and manages any extra data input not covered by an electronic chain of custody system.

Screening acts as an initial swift evaluation for drug traces. While Screening is a budget-friendly method to check for drug use in most samples, a positive Screen requires verification for court acceptance. Any samples supposedly positive in Screening necessitate a secondary confirmation.

If a sample appears positive in the Screening phase, additional hair is extracted from the initial sample and processed for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than in other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation as needed. The complete laboratory workflow from Accessioning to Confirmation is reviewed following both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 protocols.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
