What it is, How it works

Hair captures a historical record of substance use by embedding biomarkers within the hair fibers as they grow. When gathered from the scalp area, hair can reveal alcohol and drug use history for up to approximately three months. The collection of hair is straightforward, relatively resistant to tampering, and easy to transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for initial screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, a 150mg sample of hair is advisable. It's suggested to weigh the specimen with a jeweler’s scale. If scalp hair is not an option, a similar quantity of body hair may be used. When we mention head hair, it refers strictly to scalp hair. Body hair encompasses all other hair types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab's system. This step includes ensuring the proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not captured by an electronic chain of custody system.

Screening is a preliminary examination for substances of abuse. Although Screening is a cost-efficient method to rule out drug use in most samples, any positive results require confirmation to be legally valid. All samples presumed positive in Screening must undergo further confirmation.

If a sample tests presumptively positive in Screening, additional hair is extracted from the original sample to prepare for Extraction. During this phase, the concentration of drugs is extracted from the hair at much lower levels than other test methods (such as urine or oral fluid), which makes hair drug testing a complex methodology to conduct.

Any positive screening outcome is validated using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed as needed prior to confirmation. From Accessioning to Confirmation, the entire lab process is scrutinized under the CAP (College of American Pathologists) Hair designation and holds accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
