For EtG, additional tests, and/or tests exceeding 10 panels, a sample of 150mg is suggested. Utilizing a jeweler’s scale to weigh the sample is advisable. Should scalp hair be unavailable, an equivalent quantity of body hair can be obtained. The term head hair specifically refers to scalp hair, while body hair encompasses all other hair types, such as facial or axillary hair.

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step in processing the sample into a laboratory system, which involves verifying the correct sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional required data entry that is not part of an electronic chain of custody system.

Screening serves as a preliminary, efficient check for drug abuse. Although Screening effectively rules out substance use for a majority of samples at a lower cost, a confirmed positive result is necessary for validity in legal considerations. Samples with a presumptive positive result from Screening must undergo secondary confirmation.

If a sample results in a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction process. During Extraction, substances are retrieved from hair at significantly lower concentrations than alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS techniques. All samples showing presumptive positives undergo washing prior to confirmation as required. The entirety of the laboratory procedure from Accessioning through Confirmation is evaluated according to both CAP (College of American Pathologists) Hair designation and accreditation in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
