Hair follicle analyses are favored by employers with zero-tolerance drug policies, courts, and those on probation due to their longer detection window, typically up to 90 days, conducted under observation, and their robustness against tampering. However, for recent drug use within five days, urine diagnostics remain the most accurate.

What it is, How it works

Hair evaluations are now regarded as an effective method for identifying alcohol and drug consumption. Hair records a prolonged timeline of alcohol and drug intake by capturing biomarkers in the fibers of the hair as it grows. When sampled near the scalp, hair can offer roughly a 3-month detection period for alcohol and drugs. It's easy to collect, relatively tough to tamper with, and straightforward to transport.

A segment of hair measuring 1.5 inches, containing roughly 200 strands (comparable to the width of a #2 pencil) near the scalp provides 100mg of hair, the optimal sample size for testing and verification. For EtG, supplementary tests, and/or analyses exceeding 10 panels, it's suggested to use 150mg of the sample. Using a jeweler’s scale to measure the specimen is advised. If scalp hair isn't available, an equivalent quantity of body hair may be used. When we mention head hair, we mean scalp hair exclusively. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is entered into the lab’s system. This step ensures the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that an electronic chain of custody does not provide.

Screening is a preliminary check for drug presence. While it’s a cost-efficient method to dismiss drug usage in most samples, a positive screen necessitates confirmation for court admissibility. Samples that are indicative of being positive in Screening require subsequent confirmation.

If Screening suggests a positive result, additional hair is taken from the initial sample and set for Extraction. During this phase, drugs are extracted from hair in much less concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodologically.

Confirmation of any positive screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all indicative positive samples are washed as necessary. The laboratory process, from Accessioning through Confirmation, is examined under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
