What it is, How it works

Hair testing is recognized as an efficient means for identifying drug and alcohol consumption. Hair encapsulates a timeline of alcohol and drug intake by embedding biomarkers within its strands as they grow. A sample cut near the scalp can detect alcohol and drugs over an approximate three-month duration. Hair samples are easy to obtain, relatively tamper-proof, and simple to mail.

A 1.5-inch segment of roughly 200 hair strands (about the thickness of a #2 pencil) closest to the scalp yields 100mg, the perfect amount for initial and confirmatory testing. For EtG, additional tests, or panels exceeding 10, 150mg is suggested. Using a jeweler’s scale ensures accuracy in specimen weight. If no scalp hair is available, equivalent body hair may be used. "Head hair" refers to scalp hair exclusively, whereas "body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The procedure in the lab for processing a drug test result includes four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial processing of a sample into the lab’s system, which entails ensuring the sample is securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not managed by an electronic chain of custody.

Screening is a preliminary rapid evaluation for drug traces. Primarily a cost-efficient method to rule out drug use in most samples, Screening requires confirmation for a positive result to be court-acceptable. Samples testing presumptively positive need secondary confirmation.

When a sample is marked presumptively positive at Screening, additional hair is sent for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other testing methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug screening.

Confirmation of any positive result from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation if necessary. From Accessioning through Confirmation, the complete lab process is audited under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
