An ideal screening and confirmation sample comprises about 100mg from a 1.5-inch segment of approximately 200 hair strands (similar in size to a #2 pencil) adjacent to the scalp. For EtG enhancements and/or tests exceeding 10 panels, a specimen weighing 150mg is advised. It is recommended to measure the sample using a jeweler’s scale. If scalp hair is unattainable, an equivalent amount of body hair is collected. 'Head hair' specifically refers to scalp hair, whereas 'body hair' includes other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory step in integrating a sample into a lab’s system. This involves ensuring the sample's proper sealing and shipping, allocating a random Laboratory Accessioning Number (LAN), and fulfilling any supplementary data input not managed by an electronic chain of custody system.

Screening executes an initial swift inspection for substances of abuse. Although Screening is an economical method to dismiss drug usage in most samples, a confirmed positive screen is necessary for legal acceptance. Samples suspected to be positive during Screening must undergo additional confirmation.

Should a sample be deemed presumptively positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. In Extraction, drugs are isolated from hair at considerably lower concentrations than in other techniques (like urine or oral fluid), rendering hair drug analysis the most intricate technique to undertake.

Positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positive results undergo washing before confirmation if required. The full lab process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair guidelines and accredited to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
