A 1.5-inch portion consisting of around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will ensure 100mg of hair, which is the optimal portion for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the hair sample is advisable. We advise measuring the hair sample on a jeweler’s scale. Should scalp hair be unobtainable, an equivalent volume of body hair is permissible. When we mention head hair, it specifically refers to scalp hair. Body hair denotes all other hair categories (e.g., facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome involves four core stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the primary handling of a sample into a lab’s framework. This process includes confirming that the sample was properly sealed and transported, allocating a randomized LAN (Laboratory Accessioning Number), and finalizing any extra data entry not supplied by an electronic chain of custody protocol.

Screening encompasses an initial rapid examination for drugs of abuse. Although Screening is an economical approach to eliminate drug usage in a large portion of samples, a positive screen must be validated for it to be permissible in legal contexts. Any samples that yield a presumptive positive in Screening necessitate a subsequent confirmation.

Should a sample yield a presumptive positive during the Screening phase, additional hair is extracted from the initial sample for Extraction. This phase entails extracting drugs from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which renders hair drug screening the most intricate method to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before the confirmation as needed. The entire lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.
