What it is, How it works

Hair analysis has become recognized as a robust instrument for identifying substance and alcohol use. Hair retains a lasting record of alcohol and drug usage by capturing biomarkers within the strands of growing hair. When collected near the scalp, hair can offer up to around a 3-month detection timeframe for alcohol and substances. Hair is straightforward to gather, relatively tamper-resistant, and simple to transport.

A 1.5-inch sample comprising roughly 200 hair strands (similar to the dimensions of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal sample for assessment and verification. For EtG, extended panels, and/or assessments exceeding 10 panels, 150mg of the sample is suggested. We endorse measuring the sample on a jeweler’s scale. Should scalp hair be unavailable, a comparable amount of body hair can be collected. In reference to head hair, we mean scalp hair specifically. Body hair signifies all other hair types (facial hair, axillary hair, etc.).

Process Overview

The four primary stages involved in the laboratory procedure of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into the laboratory’s system. This step includes ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry missing from an electronic chain of custody system.

Screening consists of an initial rapid assessment for substances of abuse. Though Screening is an economical approach to exclude drug consumption in most samples, any positive screen must be validated to be court-admissible. Samples that are presumptively positive in Screening demand additional confirmation.

Should a sample be presumptively positive during the Screening phase, additional hair is obtained from the original sample and prepped for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumptively positive undergo washing prior to confirmation where necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the certification to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
