A 1.5-inch sample consisting of about 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, or cases exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for weighing is recommended. Should scalp hair be inaccessible, a similar quantity of body hair can be obtained. References to head hair imply scalp hair exclusively. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The four fundamental stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the early processing of a sample into a lab’s system. This process checks that the sample was correctly sealed and sent, allocates a random LAN (Laboratory Accessioning Number), and performs any additional data entry not managed by an electronic chain of custody system.

Screening involves an initial brief assessment for drugs of abuse. Though Screening effectively eliminates drug presence in most samples at a lower cost, a positive result must be confirmed for court validity. Any samples presumptively positive in Screening require additional confirmation.

Confirmation of any positive Screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as necessary. The complete lab process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation as well as ISO / IEC 17025 standard accreditation.
