A hair sample of 1.5 inches, consisting of around 200 strands (approximately equivalent to the thickness of a #2 pencil) near the scalp, will yield 100mg of hair, which is ideal for screening and confirmation. For EtG, additional tests, and/or more than 10 panel tests, a specimen of 150mg is advised. It is suggested to measure the specimen's weight using a jeweler’s scale. If head hair is not available, a comparable amount of body hair can be gathered. References to head hair specifically mean scalp hair. Body hair refers to other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into the lab's system, which involves ensuring the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in missing data not captured by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs. While Screening is an economical way to dismiss drug usage for most samples, a confirmatory test is required for a positive screen to be court-admissible. Samples that test positive in Screening need secondary confirmation.

For samples indicating a conditional positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are isolated from hair at a significantly lower concentration than other methods (such as urine or oral fluid), making hair drug screening the most challenging to conduct.

For positive results from Screening, Confirmation is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with conditional positive findings are cleaned before confirmation as necessary. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
