For EtG, additional testing, and/or assessments exceeding ten panels, 150mg of the sample is advisable. Using a jeweler’s scale for weighing the sample is suggested. Should scalp hair be unavailable, an equivalent volume of body hair can be used. References to head hair pertain only to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The processing of a drug test in a laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab’s system, ensuring proper sealing and shipping, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not incorporated by an electronic chain of custody system.

Screening performs an initial rapid evaluation for drug abuse. Though Screening is a cost-effective methodology to eliminate drug use in most samples, a confirmed positive result is necessary for legal proceedings. Any samples returning a presumptive positive in Screening require further verification.

For samples with a presumptive positive from Screening, additional hair is extracted from the main sample and subjected to the Extraction process. Here, drugs are isolated within the hair at lower concentrations than other techniques (e.g., urine or oral fluid), contributing to the complexity of hair drug screening.

Verification of positive screening outcomes is done through GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positive undergo washing before verification as applicable. The entirety of the lab's procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
