Hair offers an extended timeline of alcohol and drug usage by capturing biomarkers within its fibers as it grows. When gathered close to the scalp, hair can indicate drug and alcohol use over a 3-month period. Hair sample collection is straightforward, somewhat resistant to tampering, and easy to transport.

A sample size of 1.5 inches, consisting of roughly 200 hair strands (comparable to a #2 pencil), near the scalp provides 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or assessments beyond 10 panels, 150mg of the sample is advised. Using a jeweler's scale to weigh the sample is recommended. Should scalp hair not be accessible, a comparable quantity of body hair is acceptable. "Head hair" specifically denotes scalp hair, while "body hair" encompasses all other hair types (such as facial and axillary hair).

Process Overview

Drug test result processing in a lab encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into the laboratory system. This step includes verifying sample sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and performing data entry tasks not covered by an electronic chain of custody.

Screening serves as an initial evaluation for drug presence. Although screening offers an economical way to rule out drugs in most samples, any positive findings need confirmation for court purposes. Samples with positive screening results necessitate subsequent confirmation.

For samples with positive results in Screening, additional hair is drawn from the original specimen for the Extraction phase. In this step, drugs are isolated from hair at minimal concentrations, making hair drug screening complex compared to other methods like urine or oral fluid testing.

Confirmation for positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if needed. The complete process, from Accessioning to Confirmation, is overseen by the CAP (College of American Pathologists) Hair services and complies with ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
