A 1.5-inch sample consisting of about 200 hair strands (about the width of a #2 pencil) from near the scalp will yield 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional items, and/or tests exceeding 10 panels, it's advised to use 150mg of the sample. We recommend measuring the sample with a jeweler’s scale. Should scalp hair be inaccessible, a similar quantity of body hair can be used. When referencing head hair, it specifically pertains to scalp hair. Body hair encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The drug test result processing in the lab comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the foundational processing of a sample within a lab's system. This phase ensures that the sample was appropriately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and accomplishes any supplementary data entry absent from an electronic chain of custody system.

Screening incorporates an initial brief evaluation for drugs of misuse. While Screening serves as an economical approach to exclude drug consumption in most samples, a positive result necessitates confirmation to be court-admissible. Any samples presumed positive in Screening require further confirmation.

If a sample is anticipated positive in the Screening phase, additional hair is taken from the initial specimen and readied for Extraction. During this phase, drugs are isolated from hair at markedly lower concentrations than seen in other techniques (e.g., urine or oral fluid), rendering hair drug screening as the most challenging technique to execute.

Confirmation of any positive Screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed prior to confirmation when essential. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
