For EtG, supplementary tests, and/or those exceeding 10 panels, a specimen amounting to 150mg is advised. It's suggested to weigh the specimen using a jeweler’s scale. If scalp hair cannot be collected, a similar quantity of body hair is acceptable. The term head hair specifically pertains to scalp hair, whereas body hair refers to all other types, such as facial or axillary hair.

Process Overview

The four key stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a laboratory’s system. This process consists of verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening consists of a preliminary swift check for the presence of drugs. While Screening is cost-efficient for ruling out drug use in most samples, a positive screening requires confirmation to be considered admissible in legal proceedings. Any samples presumptively positive during Screening necessitate secondary confirmation.

Should a sample test presumptively positive during the Screening phase, additional hair is extracted from the initial specimen for the Extraction phase. At this stage, drugs are extracted from hair at significantly lower concentrations compared to alternative methods (e.g., urine or oral fluid), making hair drug screening one of the most complex techniques.

Confirmation of any positive screening is carried out via GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive samples are washed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
