Hair testing is now regarded as an effective means for identifying drug and alcohol consumption. Hair maintains a comprehensive record of alcohol and other drugs by embedding biomarkers within the fibers of the developing hair strand. When taken near the scalp, hair can offer around a 3-month detection span for alcohol and other substances. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A hair specimen of 1.5 inches, comprising approximately 200 strands (around the size of a #2 pencil), near the scalp yields 100mg of hair, which is the optimal sample for analysis. For EtG, additional tests, and/or over 10 panels, 150mg of the sample is suggested. We suggest measuring the sample on a jeweler's scale. If scalp hair is not available, a comparable amount of body hair may be obtained. When discussing head hair, we mean scalp hair exclusively. Body hair refers to all other hair kinds (facial, armpit, etc.).

Process Overview

The laboratory processing of a drug test result is comprised of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the introductory processing of a sample into a lab's setup. This covers ensuring that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finishing up any extra data entry absent from an electronic chain of custody system.

During this stage, drugs are derived from hair at a much smaller concentration than in alternative techniques (e.g., urine or oral fluid), hence why hair drug screening is the most challenging technique to execute.

Verification of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed as needed before the confirmation step. The full lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
