When taken from near the scalp, hair offers a detection period of around three months for both alcohol and other drugs. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A segment measuring 1.5 inches of roughly 200 hair strands (comparable in thickness to a #2 pencil) obtained nearest to the scalp amounts to 100mg of hair, the optimal sample for both screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, a specimen weighing 150mg is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not present, a similar quantity of body hair is acceptable. When referencing head hair, we mean scalp hair only. Body hair encompasses all other types including facial and axillary hair.

Process Overview

The process within the laboratory for determining drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into the lab’s system. This stage includes confirming that the sample has been properly sealed and transported, the assignment of a randomized LAN (Laboratory Accessioning Number), and completing any further data entry absent in an electronic custody system.

Screening represents an initial rapid assessment for drugs of abuse. Although it serves as a cost-efficient method to eliminate drug usage in most samples, any positive screen requires confirmation to be permissible in legal contexts. Samples with presumptive positives from Screening need a subsequent confirmation.

Should a sample return a presumptive positive after Screening, additional hair is extracted from the sample for the Extraction phase. Here, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), demonstrating why hair drug screening is considered the most complex technique to execute.

Confirmation of any positive screening outcome uses GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair category and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
