For EtG, add-ons, or tests exceeding 10 panels, a sample of 150mg is advised. Utilizing a jeweler’s scale for weighing the sample is suggested. If scalp hair is lacking, an equivalent amount of body hair may be used. Reference to head hair pertains specifically to scalp hair, whereas body hair encompasses all other types (such as facial or axillary).

Process Overview

The laboratory examination of a drug test result involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing within a laboratory's system. This encompasses verifying the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry absent from an electronic chain of custody system.

Screening consists of a preliminary rapid assessment for potential drug abuse. While Screening is efficient for excluding drug use in the majority of samples, a positive screen must be confirmed for courtroom admissibility. Presumptive positives from Screening demand secondary confirmation.

In cases of a presumptive positive at the Screening stage, additional hair is extracted from the initial sample for Preparation in the Extraction process. Drugs are removed from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), establishing hair drug analysis as the most challenging procedure.

Confirmations for any positive screenings are managed through GC/MS, GC/MS/MS or LC/MS/MS. Prior to confirmation, all samples presumed positive undergo necessary washing. The laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
