For EtG, additional tests, or those exceeding 10 panels, a 150mg specimen is suggested. Utilizing a jeweler’s scale to weigh the specimen is advisable. If scalp hair is not available, an equivalent quantity of body hair can be utilized. The term head hair specifically refers to scalp hair, while body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The four fundamental steps for processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial stage of integrating a sample into a laboratory’s system. This entails confirming the proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input that an electronic chain of custody system does not provide.

Screening includes a preliminary rapid test for drugs of abuse. Although Screening is a budget-friendly means to exclude drug use in most samples, a positive result needs verification for legal admissibility. Any samples presumed positive in Screening necessitate further confirmation.

If Screening indicates a presumptive positive, additional hair is taken from the original specimen for Extraction. During this phase, drugs are extracted from hair at significantly lesser concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging technique to conduct.

Verification of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation if required. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
