However, for detecting drug intake in the past 5 days, urine tests remain the most precise.

What it is, How it works

Hair testing is now recognized as an effective method for detecting drug and alcohol consumption. Hair captures a long-term drug and alcohol history by embedding biomarkers within the hair fibers as they grow. When taken near the scalp, hair can offer a detection timeframe for alcohol and other substances of up to roughly three months. Hair collection is straightforward, relatively resistant to tampering, and can be shipped with ease.

A 1.5-inch segment consisting of nearly 200 hair strands (approximately the width of a #2 pencil) closest to the scalp provides 100mg of hair, which is optimal for screening and confirmation. For EtG, additional assessments, and/or tests exceeding 10 panels, a sample of 150mg is advised. We suggest using a jeweler’s scale to measure the sample weight. If scalp hair is not an option, an equivalent amount of body hair can be used. In this context, head hair specifically denotes scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the early handling of a sample into the lab’s system. It involves verifying the sample’s proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not captured by an electronic chain of custody system.

Screening consists of a preliminarily quick check for drugs of abuse. Although Screening is a cost-effective approach to exclude drug use in most samples, a positive result must be confirmed to be considered valid in court. Samples that test positive in Screening require secondary confirmation.

If a sample records a presumptive positive during Screening, additional hair is taken from the initial specimen for Extraction preparation. In this phase, drugs are removed from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology.

GC/MS, GC/MS/MS, or LC/MS/MS methods are used for the Confirmation of any positive screening outcomes. Before confirmation, all presumptive positive samples undergo washing if necessary. The entire process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

  • Difficulty to cheat: It is extremely challenging to manipulate a hair drug test, yielding more accurate results.

  • Cannot detect recent use: Drugs become identifiable in hair after about 5-7 days.
  • Cost: Hair drug tests usually cost more than alternative drug testing techniques.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
