A 1.5-inch segment of roughly 200 hairs (approximately the size of a #2 pencil) nearest to the scalp provides 100mg, the preferred amount for screening and verification. For EtG, additional tests, and/or analyses exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale is recommended for weighing the sample. Should scalp hair be unavailable, an equivalent quantity of body hair can be used. In this context, head hair specifically refers to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The core stages in the laboratory handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab’s framework. This process involves checking the sample’s seal and shipment, assigning it a LAN (Laboratory Accessioning Number), and completing any supplemental data entry missing from an electronic chain of custody.

Screening serves as a preliminary assessment for substances of abuse. While it offers a cost-efficient way to dismiss drug use for most samples, any positive screen must be substantiated for legal credibility. Samples yielding a presumptively positive result in Screening necessitate secondary confirmation.

Should a sample show a presumptive positive in the Screening phase, more hair is drawn from the initial batch for Extraction. During this step, substances are removed from hair at much lower concentrations than alternative methods (e.g., urine or oral fluid), rendering hair drug analysis the most challenging to execute.

Any positive screening outcome undergoes verification through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positives receive washing prior to confirmation when necessary. The entire verification chain from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair safety and ISO / IEC 17025 compliance benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
