What it is, How it works

When hair is collected near the scalp, it can offer a detection span of up to about 3 months for alcohol and drugs. Collecting hair is straightforward, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches with approximately 200 strands of hair (comparable to a #2 pencil's circumference) closest to the scalp will accumulate 100mg—the perfect amount for testing and confirmation. For EtG, additional tests, and/or examinations beyond 10 panels, 150mg of hair is advised. We suggest quantifying the sample using a jeweler's scale. If scalp hair isn't available, a similar amount of body hair can be gathered. When discussing head hair, we mean only scalp hair. Body hair encompasses all other types (like facial, underarm, etc.).

Process Overview

The four primary steps in the laboratory processing of hair drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a lab's framework. This process ensures the sample was properly sealed and sent, assigns a random Laboratory Accessioning Number (LAN), and finalizes any supplementary data entry absent from an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While Screening is an economical method to eliminate drug usage in most samples, a positive result must be confirmed to be valid in legal proceedings. Samples with an indicative positive in Screening need further confirmation.

If a sample shows a tentative positive in the Screening phase, additional hair is extracted from the initial sample for preparation in the Extraction process. Here, drugs are obtained from hair at much lower concentrations than other methods (such as urine or oral fluids), which is why hair drug testing is amongst the most complex methods to perform.

Confirmation of any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes are cleansed before confirmation if necessary. The process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair category and accredited to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
