What it is, How it works

Over an extended period, hair captures a history of alcohol and drug markers by embedding these biomarkers within the fibers of the hair as it grows. A sample taken near the scalp can detect alcohol and drugs for about a three-month span. Hair is easy to collect, hard to tamper with, and convenient to transport.

A 1.5-inch strand collection (around 200 strands, similar in size to a #2 pencil) taken from the scalp yields 100mg, which is perfect for preliminary and confirmatory testing. For EtG, additional tests, or panels exceeding 10, 150mg of hair is advisable. It is suggested to measure the hair on a jeweler’s scale. If scalp hair isn't accessible, the equivalent quantity of body hair could be used. "Head hair" pertains specifically to scalp hair, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test consists of four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within a lab’s system. This phase verifies the sample's proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and concludes any extra data input lacking in an electronic chain of custody framework.

Screening executes an initial rapid evaluation for drug abuse. While cost-effective, screening serves primarily to eliminate drug usage in most samples, necessitating confirmation for court admissibility. Samples that demonstrate a tentative positive in Screening require additional confirmation.

Should a sample yield a positive indication in Screening, additional hair is extracted from the initial sample for Extraction. During Extraction, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), underlining the challenge presented by hair drug screening.

Confirmation of any positive results from screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples presumed positive undergo washing if necessary. The complete process, from Accessioning to Confirmation, undergoes review according to both the CAP (College of American Pathologists) Hair assignment and its accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
