What it is, How it works

Hair analysis has emerged as an effective method for identifying substance use. It offers a long-term overview of alcohol and drug use by capturing biomarkers within the fibers of a growing hair strand. When taken near the scalp, hair can offer insights into up to roughly a 3-month period for substance detection. Hair collection is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch portion of about 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred sample for analysis and verification. For EtG, add-ons, and tests beyond 10 panels, 150mg of the sample is suggested. It's advised to weigh the sample with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair could be used. When mentioning head hair, we refer only to scalp hair. Body hair entails all other types (facial, axillary, etc.).

Process Overview

There are four primary stages involved in lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary handling of a sample within a lab’s system. This process includes confirming the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening includes an initial fast test for substance abuse. While Screening is a cost-effective approach to eliminate most samples, a positive screen needs affirmation for courtroom admissibility. Any samples that are presumptive positives in Screening mandate additional confirmation.

If a sample indicates a presumptive positive during the Screening phase, more hair is extracted from the initial sample and readied for Extraction. In this step, substances are withdrawn from the hair at significantly lower concentrations than in other methods (ex. urine or saliva), explaining why hair drug testing is a complex approach.

Confirmation of each positive screening outcome happens through GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample is cleaned before confirmation if essential. The entire lab process from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
