A 1.5-inch piece comprising roughly 200 hair strands (similar in size to a #2 pencil) closest to the scalp will yield 100mg, which is optimal for both screening and confirmation processes. For EtG, add-ons, and/or examinations exceeding 10 panels, a sample of 150mg is preferred. It is advised to measure the specimen using a jeweler’s scale. If scalp hair can’t be obtained, an equivalent quantity of body hair can be used. Referring to head hair specifically pertains to scalp hair. Body hair encompasses other hair types (such as facial and axillary hair).

Process Overview

The drug test laboratory procedure consists of four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is incorporated into a lab's system. This step includes confirmation that the sample was properly sealed and sent, allocation of a random LAN (Laboratory Accessioning Number), and completion of any supplementary data entry not available from an electronic chain of custody system.

Screening is an initial rapid evaluation for drug abuse. Although Screening efficiently rules out drug use in most samples, a positive screen necessitates confirmation to be legally permissible. Any samples flagged as presumptively positive in Screening require a subsequent confirmation.

If a sample tests as presumptively positive during Screening, additional hair is taken from the original sample for Extraction. In this phase, drugs are extracted from hair in much lower concentrations compared to methods like urine or oral fluid testing, making hair drug testing particularly challenging.

Confirmation of any positive screening results occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
