A 1.5-inch section of approximately 200 hair strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is advised. It's best to determine the specimen's weight using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair is acceptable. The term "head hair" refers solely to scalp hair, whereas "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The core stages in processing a drug test result at the laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial introduction of a sample into the laboratory's system. This step involves confirming that the sample arrived sealed and in proper condition, assigning it a unique LAN (Laboratory Accessioning Number), and performing any supplemental data entry required when the information is not available through an electronic chain of custody system.

Screening entails an initial rapid evaluation for substances of abuse. Although Screening is a cost-efficient approach to ruling out drug use in most samples, any identified positive must be verified to be legally defensible. Samples that show presumptive positives in Screening necessitate a subsequent confirmation.

In instances where a sample is presumptive positive during Screening, additional hair is taken from the initial sample and made ready for Extraction. During this phase, drugs are extracted from the hair at concentrations significantly lower than those used in other methods (e.g., urine or oral fluid), hence making hair drug screening the most complex method to perform.

Verification of any positive Screening findings is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation if necessary. The complete lab process, from Accessioning through to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
