For EtG, additional tests, or panels exceeding 10, 150mg is suggested. It's advised to measure the sample with a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. In reference to head hair, it pertains strictly to scalp hair. Body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The laboratory process for analyzing a drug test outcome includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into a lab's system. This step involves checking that the sample is sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody procedure.

Screening involves a preliminary examination for drug abuse. While it is a cost-efficient method for ruling out drug usage in the majority of samples, any positive results from Screening must be confirmed to hold up in legal settings. Samples returning a presumptive positive in Screening require further confirmation.

In the event of a presumptive positive result in Screening, additional hair is taken from the original sample for Extraction preparation. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening findings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned before Confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
