What it is, How it works

Hair retains a prolonged chronicle of alcohol and other drugs by ensnaring biomarkers in the hairs of the developing strand. When procured near the scalp, hair can offer roughly a 3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and conveniently transportable.

A sample of 1.5 inches consisting of about 200 strands of hair (comparable to a #2 pencil) close to the scalp yields 100mg of hair, an ideal sample for analysis and validation. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advisable. We suggest using a jeweler’s scale for precise measurement of the specimen. If scalp hair isn't available, a corresponding quantity of body hair can be obtained. By head hair, we mean scalp hair strictly. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary integration of a sample into a lab’s workflow. It includes verifying that the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening consists of a preliminary swift assessment for drugs of abuse. Screening is an economical technique to eliminate drug usage in most samples, yet a positive screen necessitates confirmation to be accepted in court. Any samples presumed positive during Screening require additional verification.

If Screening indicates a presumptively positive sample, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most intricate methodology to execute.

Positive screening results are affirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
