A 1.5-inch hair sample consisting of around 200 strands (resembling the thickness of a #2 pencil) near the scalp equals 100mg, which is the optimal quantity for both screening and confirmation. For EtG, optional tests, or panels exceeding 10, 150mg of the specimen is advisable. We suggest using a jeweler’s scale for accuracy. If scalp hair isn't available, a comparable amount of body hair can be used. References to head hair strictly mean scalp hair, while body hair covers all other varieties (e.g., facial, axillary).

Process Overview

The procedure in labs for analyzing drug test outcomes includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the sample's induction into the laboratory’s system, ensuring its seal and shipment integrity, allocating a unique LAN (Laboratory Accessioning Number), and entering any missing data not captured by an electronic chain of custody.

Screening acts as the preliminary detection for drug presence. Although it is an economical method to eliminate drug use in most cases, a screen that yields a positive signal necessitates confirmation for legal admissibility. Any suspect positive samples require subsequent verification.

Upon encountering a suspect positive in Screening, additional hair from the initial sample is extracted and prepared for further analysis. Here, drugs are extracted from hair at much lower levels than alternative methods (like urine or oral fluids), making hair drug testing notably challenging.

Positive screen confirmations are performed using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive is cleansed pre-confirmation as needed. The laboratory workflow from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accredited as per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
