A sample of 1.5 inches consisting of approximately 200 hair strands (nearly the dimension of a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect sample for screening and verification. For EtG, additional tests, and/or analyses beyond 10 panels, 150mg of the specimen is preferred. It is advised to weigh the specimen on a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair may be gathered. When we mention head hair, we refer solely to scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The process of generating a drug test result in the laboratory consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab’s system. This encompasses confirming the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any supplemental data entry not supplied by an electronic chain of custody method.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a practical means to exclude drug usage in most samples, a positive screen must be confirmed to stand in legal proceedings. Any samples that test presumptively positive in Screening necessitate a follow-up confirmation.

If a sample shows a presumptively positive result in the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (such as urine or oral fluid), which is why hair drug screening poses the greatest challenge to execute.

Confirmation of any positive screening result occurs using GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample undergoes washing prior to confirmation if required. The full lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
