Screening includes a rapid preliminary evaluation for abuse substances. Although it offers a cost-efficient method to exclude drug usage in most specimens, a positive outcome from Screening must undergo confirmation to be court-acceptable. Screens that show a presumptive positive result require further verification.

For samples deemed presumptively positive during Screening, additional hair is extracted from the initial collection and readied for Extraction. During this phase, drugs are isolated from the hair at concentrations notably lower than those found in other testing methods (such as urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive screen results is carried out utilizing GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples returning presumptive positive results undergo washing before confirmation if needed. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair certification and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
