When collected near the scalp, hair can reveal a detection period of roughly three months for alcohol and drugs. Hair is easy to gather, somewhat hard to tamper with, and simple to transport.

A sample measuring 1.5 inches consisting of about 200 hair strands (about the diameter of a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal amount for preliminary and confirmatory testing. For EtG, additional tests, and/or tests exceeding 10 panels, it is suggested to collect 150mg of the sample. We recommend weighing the sample with a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair may be used. When referencing head hair, it pertains solely to scalp hair. Body hair refers to any other type of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample within a laboratory's system. It includes validating that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening consists of a preliminary fast check for substances of abuse. Although Screening is an economical approach to eliminate drug usage for most samples, a positive result requires confirmation to be admissible in judicial settings. Samples that test presumptively positive during Screening necessitate secondary confirmation.

If a sample tests presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. During this phase, drugs are extracted from hair at concentrations much lower compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptively positive undergo washing prior to confirmation where necessary. The entire laboratory process from Accessioning through Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
