A sample of 1.5 inches, consisting of around 200 hair strands (approximately the width of a #2 pencil), close to the scalp will yield 100mg of hair, which is the perfect sample for both screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, a specimen weight of 150mg is suggested. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The four critical steps in a laboratory's processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial insertion of a sample into a lab’s system. This step verifies the sample's proper sealing and shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any further data entry not provided by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage in most samples, a positive screen requires confirmation for it to be valid in court. Any samples that are presumptively positive in Screening need a secondary confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the initial sample and prepared for Extraction. During this step, drugs are isolated from hair at significantly lower concentrations than in other methods (e.g., urine or saliva), making hair drug screening a complex process.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if needed. The laboratory process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
